Referral Program Guidelines

We’re excited to have you share the gift of health and wellness with others! To ensure everything runs smoothly, please review our referral program guidelines:

1. Referral Form Completion: The referred patient must complete our Surgery Approval Form.
2. Acknowledging the Referrer: On the form, in the “Who referred you?” field, the referred patient must clearly write your name.
3. Surgery Completion: The referred patient must successfully undergo the surgery and complete payment.
4. First Time Gastric Sleeve Referral Reward: A referral fee of $100 will be sent via Zelle or Venmo one week after the patient’s procedure.
5. Gastric Bypass, Mini Gastric Bypass and Revision Surgery Referral Reward: A referral fee of $150 will be sent via Zelle or Venmo one week after the patient’s procedure.
